Tuition Assistance for St Monica Parish Families

  • Must be a St. Monica Catholic Church Parishioner in good standing
  • Kindergarten through 8th grade students only, Preschool students are not eligible
  • No family receives 100% tuition assistance
  • Application must be completed by the individual paying tuition
  • Application must be completely filled out and all requested information provided by the due date

FACTS Grant & Aid Assessment will conduct the financial need analysis. The following information is required in order for FACTS to process your application:

2. Submit 2016 documents either by uploading in your online account or faxing to 866-315-9264: 
  • Copies of your IRS Federal Form 1040, 1040A, or 1040EZ U.S. Individual Income Tax Return. If applicant and co-applicant file separately, we require both tax returns for the same tax year.
  • Copies of all W-2 Wage and Tax Statements for both the applicant and co-applicant
    Copies of all supporting tax schedules
  • Copies of all supporting documentation for household Non-Taxable Income such as Social Security Income, Welfare, Child Support, Food Stamps, Workers’ Compensation, and Temporary Assistance for Needy Families (TANF)
The applicant is responsible for following up with FACTS and ensuring that all required documentation has been received in a timely manner. NOTE: Award decisions are not made by FACTS, they are made by the St Monica Tuition Assistance Committee. The Committee will review applications and will approve or deny each Tuition Assistance Application based on the calculated financial need of the applicant. Notification of assistance will be made on May 15.
